Case Manager Job description
Qualifications:
- Bachelor's or Graduate degree
- 21 years of age or older
- One year or more working with troubled youth.
- Valid driver's license
- Background check
- Drug test
Duties and Responsibilities:
- Participate in youth intake and assessment interviews.
- Maintain youth files and develop goal-oriented service plans.
- Maintain confidentiality for all youth and program operations.
- Provide and maintain appropriate documentation.
- Establish helping relationships with youths and families/guardians.
- Provide advocacy as specified by Right at Home and contract guidelines.
- Facilitate and participate in individual and group counseling.
- Assist in planning and executing group, program, & recreational activities
- Prepare and submit documentation, assessments and reports within required timelines.
- Provide crisis intervention for youth and families.
- Attend and participate in training and workshops.
- Maintain professional and ethical standards.
- Comply with both the agency and program policy and procedure manuals.
- Comply with all Licensing Standards
Direct Care staff job description
Qualifications:
- High school diploma or GED
- 21 years of age or older
- 6 months; (a year or more) experience working with troubled youth preferred.
- Valid driver's license
- Valid insurance for staff transporting clients.
- Background check
- Drug test
Duties and Responsibilities:
- Serve as a positive role model to youth.
- Maintain work environments free of recognized hazards/Provide care, welfare, safety and security.
- Provide transportation to the residents for appointments as needed.
- Serve meals to each resident according to the menu.
- Provide help with homework, routine hair/skin care, laundering, cooking, housekeeping, and shopping.
- Help develop skills and personal goals.
- Distribute medicine to clients as needed, documenting in the medicine log.
